The Magnolia School District, in conjunction with the Arkansas Department of Health, will hold a flu immunization clinic at each campus.  There will be a flu clinic on Tuesday, October 18th at East Side Elementary School; that’s for children in kindergarten through the 3rd grade. There will be a clinic at Central Elementary School and Magnolia Junior High on Wednesday, October 19th. The flu clinics for Walker Pre-Kindergarten Center and Magnolia High School will be on Thursday, October 20th. Students on all campuses will receive information packets; they include forms which must be completed and returned to the school if the child is to receive the immunization. Parents must complete the 2016 Influenza Immunization Consent Form and the Family Education Right to Privacy Act Form (FERPA).  All forms need to be filled out completely on both sides and returned to the appropriate campus by this Friday, September 23rd. Parents also have the option to submit the forms online at the Arkansas Department of Health website, https://ADHSchoolFluPreReg.Arkansas.gov. Students without the completed forms will not receive the flu immunization. For more information, you can call the Columbia County Health Unit.
